Did you check your directory permissions.
Make sure the upload directory has 775 CHMOD.

yes, please set permissions 755 for 2 file fileuploader.php and uploader.php

i've changed the directory permission for fileuploader.php and uploader.php to 755.
but it still not working. it still shows, uplaod failed. image undefined.

Any suggestions/guidance on what to do?

What browser are you using.
Did you set your joomla directory CHMOD permissions right?

Is this directory CHMOD 755: modules/mod_btslideshow/images

im using chrome.
and yes, i've changed the directory for the below folder n files to 755 :

modules/mod_btslideshow/images

modules/mod_btslideshow/fileuploader.php

modules/mod_btslideshow/uploader.php
