You must create a link to the contact form. Menu-MainMenu->New Link

I feel quite embarrassed as it seems like a easy fix but i still don't understand what the link should be?

A article, external url, K2 item, k2 catogory etc?

I don't understand how that particular contact and map page will show up on my website?

Could you please give me a detailed simple instructions and example so I can sort it one and for all lol.

You should read better all options, you need single contact, search for contact options...
