More information about this error may be available in the server error log.

Check to know what exactly is error, then to fix.

this was the error found by the server

As verified by accessing the logs of submitting your form, below the actual error:

"malformed header from 'index.php' script: Bad header: More than one" from "person, referer: contabilidadepempreendedor.com.br/index.php/contact-us"

The above error would be regarding the configuration of your form
